Duplicate Entries viewing issues

I tried looking for "dupes" in the Issue Status or Type lookups and found none. I looked at the lookup up tables (I'm assumimg these are the ones ending in lut) and I only found one table that looks suspect, but I'm not sure.  The table is : issuestatetransitionlut

I checked those tables and all seems fine.  The only remote thing I can see is that the image file in the "issuetypelut" table is used more than once.  Below are all the tables in question:

Your issue type lookup contains duplicate typeid (5). Change it.
