Custom "field" during changing status

I would like to show some custom field during changing status (for example a want check box and comment insert memo when someone "test" issue and say it's ready for production). Is this possible with Gemini?

If I wasn't clear enough...
When I change "some status to another" i want some "form" between. For example when someone change status from "in work" into "in test" I want form with "custo fields":
checkbox-done this1
checkbox-done this2
memo-comment

And then after chenging into some other status (for example into "tested" state into "resolved"):
checkbox - tested on this server
checkbox - tested on this platform
checkbox - I agree to move this feature into production...
memo  - comment

What do you mean with answer? Is this feature which is available when I buy source code or when I buy source code I have to "program" this feature myself?
