Weird -- Issues are in Triplicate After Updating Issue

I have a strange issue that seemed to have cropped up in v2.0.2. I upgraded to v2.0.3, but it still exists.  It seems like after I edit an issue (set it to another release or change something on it -- have not narrowed which exactly causes that yet), the issue shows up 3 times in the Issue list.  The Issue ID is the same for all three.  I did a query in the Issue table and there is only 1 instance for that Issue ID.  Could there be a many to many relationship somewhere in the DB where the query should have a "DISTINCT" clause to it or something?

Did you change the priority /status or any other lookup tables?

Please check the the lookups do not have duplicate entries.

No, we did not change anything in any of the canned lookup tables.

After further investigation, it appears that there is a row for the issue for three of the Issue Types which all had the same ID.  We must not have added a unique ID when we added those additional types in.
