Test plan edit dialog not functional

When attempting to edit the fields on test plan or test case, the fields in the edit dialog is scrambled. Using v5.1.5.3639 and a Chrome browser.

In IE9, the dialog does not appear at all.

More info - This appears to be a side affect of clicking the "+" to add a test case from the Test Plan page. The dialog there is non-scrollable. After that, the other dialogs are messed up.

One workaround I have found is to return to the main Items page and drill back down to the test plan page.

This is reproducable.
